Hours of Work: 35 hours per week

Purpose of the Position:
We are seeking a dedicated and compassionate Case Manager to support individuals living with addictions and/or mental illness in the Districts of Muskoka and Parry Sound. This role involves carrying out case management, psychosocial rehabilitation, crisis intervention, and treatment. The ideal candidate will have the clinical skills and experience necessary to assess, plan, develop, coordinate, and provide treatment through rehabilitative and supportive services in a multidisciplinary and/or collaborative environment.

Education and Qualifications:
Bachelor’s degree in a behavioral science or equivalent (equivalency for the purpose of qualification means a university education in the social sciences, which includes or is supplemented by practical courses in these disciplines)
Registration, membership, or application for membership in the college appropriate to your discipline is an asset
Knowledge and awareness of Indigenous culture and languages considered an asset
Knowledge of Indigenous populations within the traditional territories of Muskoka and Parry Sound
Ability to identify and describe a range of traditional and non-traditional healing and wellness practices and sources of expertise including elders and healers
Experience providing case management services to those experiencing addictions/mental illness
Familiarity with Psychosocial Rehabilitation principles and practices and the concept of recovery is an asset
A commitment to the rights of those experiencing addictions/mental illness
Ability to work autonomously as well as cooperatively within a team environment in a rural setting
Strong screening and assessment skills to meet the needs of the client
Knowledge and skill in providing brief services and support to clients
Responsible for providing care/service to clients in areas related to various programs including outreach, individualized assessment and planning, direct service, monitoring and evaluation, and advocacy
Excellent communication, organization, and interpersonal skills
Working knowledge of Microsoft Windows, the Internet, and data management software
A willingness to serve clients within the communities of the District of Muskoka and Parry Sound
A valid Ontario G or G2 driver’s license and a safe and reliable means of transportation
Bilingual in French/English language considered an asset
Work schedule will include days/evenings/weekends and statutory holidays
Willingness to work after-hours and on-call
Salary Scale: $28.41 to $37.20 per hour commensurate with qualifications and experience
Benefits: Comprehensive health benefit plan after six months of employment and Registered Retirement plan with HOOPP where contributions start on the date of hire
